2017-18 Health Requirements

PreSchool – Required immunizations

PrepK –Required immunizations  (same as K)

Kindergarten – Vision screen certificate, Dental screen certificate, required immunizations

3rd grade – Vision screen certificate

7th grade – Tdap & NEW MenACWY vaccine (1 dose) if DOB is after 9/15/04 BY FIRST DAY OF SCHOOL ***NO GRACE PERIOD GRANTED BY LAW***

9TH GRADE – Dental certificate

12TH GRADE – NEW MenACWY vaccine (2 doses, or 1 if given on or after age 16) if DOB is after 9/15/99 BY FIRST DAY OF SCHOOL***NO GRACE PERIOD GRANTED BY LAW***

-As of 4/24/17 there were 25 current 6th graders needing the required Tdap and new meningococcal vaccine before entering 7th grade.
-As of 4/24/17 there were 28 current 11th graders needing the required new meningococcal vaccine before entering their senior year.
-Public Health came and gave vaccine to 14 students who turned in permission forms and qualified.  **PLEASE REMEMBER NO GRACE PERIOD IS ALLOWED FOR THE NEW VACCINE REQUIREMENTS.**  I have access to IRIS, the statewide database and can look up and print the records.  Please contact me if you don’t know if your child is compliant or not and I can tell you at once from the list. If you already have a waiver in place, you do NOT need to provide a new one.

Background Information:

For the 2017/18 school year, and all future school years, all students entering into 7th grade and born after 9/15/2004, will need proof of one dose of meningococcal conjugate vaccine (MenACWY).  All students entering into 12th grade, and born after 9/15/1999, will need proof of 2 doses of meningococcal (A,C,W,Y) vaccine; or 1 dose if received when the student was 16 years of age or older.

Meningococcal conjugate vaccine (MenACWY) provides protection against 4 serogroups (A, C, W, & Y) that cause serious, life-threatening illness. Meningococcal bacteria are spread via upper respiratory droplets, such as saliva.  Mortality is about 10-15% even with appropriate treatment.  Of those who recover, up to 20% suffer from serious after-effects such as permanent hearing loss, limb loss, or brain damage.  Meningococcal vaccines are safe and have been found to be 85-100% effective in preventing infection from these 4 subtypes. ***PLEASE NOTE:  Per Iowa Law, THERE IS NO GRACE/EXTENSION PERIOD for implementation of this requirement.  The proof MUST be presented by the start of the school year in order to be enrolled in school.  You may present a Certificate of Exemption (Medical from a physician; or notarized religious waiver), if you so desire.  If your child already has an exemption on file, you do NOT need a new/additional one.
